Thank you for joining us for a summer of fun at Imagination Stage

Each young person MUST have a completed set of forms filed with Imagination Stage before camp begins. The DEADLINE IS JUNE 1st, and we've made it EASY, FAST and ENVIRONMENTALLY FRIENDLY. Each camp requires a slightly different set of forms (see the links to the required forms for YOUR camp below).

Suggestions for making your form completion easier: (NOTE: users are having trouble with saving the forms, please print until further notice)

While every camper needs a complete set of forms, we suggest completing them for one camper then editing and printing the changes for each additional camper. (if your family is attending more than one camp, eg Arts Express and Arts Adventure, please make sure ALL forms for appropriate camps are completed.)
Finally, if your young person is attending more than one camp, please make sure you complete all the forms for each camp once.
